Scheduling and Safety Questionnaire

Submit the complete Scheduling and Safety Questionnaire form

The form should be filled out when the spokesperson of an approved experiment is ready to run the experiment – at least 6 months prior to the envisioned run date. The form is intended to help in establishing the division of responsibility between individual experimental groups and NSCL for the conduct of experiments and to identify the lab resources needed for your experiment. The submission of the questionnaire triggers the start of scheduling your approved experiment. The questionnaire will be sent to Jill Berryman, the Manager for User Relations, and the spokesperson should feel free to contact Jill with any scheduling questions (useroffice@nscl.msu.edu).
